In the face of escalating hospital capacity challenges and staffing issues across the country, effective patient flow management has become paramount. This panel presentation, featuring Chief Medical Informatics Officers and Data Scientists from M Health Fairview, Essentia Health, UC San Diego, Washington University, and Hennepin Health, will delve into innovative strategies, IT/informatics tools and workflows that are allowing optimized patient flow at their organizations. The discussion will highlight several real-world examples of innovative IT solutions to optimize inpatient flow. System operation centers can facilitate real-time decision-making for patient movement and resource allocation in a way that receives outside patient information, coordinates flow in the system and aligns staffing to meet the level of care and medical needs of patients. Discharge planning tools help staff plan for and coordinate discharges, emphasizing the importance of preparing for discharge early, using automatic milestone tracking to address potential barriers to discharge, and coordinating transitions to post-acute care facilities. A perioperative telemedicine care model uses machine learning risk forecasting to facilitate real-time care management and optimization of surgical patient flow. By fostering collaboration between hospitals and outpatient facilities, health systems can keep patients out of the hospital whenever possible, reduce readmissions, and ensure seamless transitions from hospital to clinic. This approach alleviates pressure on hospital capacity, promotes continuity, and improves overall patient satisfaction.
