Patient-facing decision support systems (PDSS) enhance patient engagement by integrating individuals into their care decisions. While clinical decision support systems are widely used, PDSS remains underutilized. We developed a mobile health application based on the World Health Organization’s Integrated Care for Older People (ICOPE) framework to empower seniors in New Mexico. This PDSS allows self-assessment of intrinsic capacities, provides personalized recommendations, and facilitates early intervention, promoting informed decision-making and proactive health management for aging populations.

HepPlus is a nationwide digital health platform integrating an e-registry, clinical decision support (CDS), and e-consultation system to enhance screening and treatment of viral hepatitis B and C and hepatocellular carcinoma (HCC). The system streamlines patient identification, risk stratification, and specialist consultations, aiming to reduce disease burden and improve care coordination. This demonstration will showcase HepPlus’s key functionalities, its impact on screening workflows, and its scalability for broader public health applications.

The resurgence of syphilis in the United States presents a significant public health challenge. Much of the information needed for syphilis surveillance resides in electronic health records (EHRs) in both structured and unstructured form. For this systems demonstration, we will present an open-source public health surveillance registry platform that integrates FHIR, OMOP, and generative AI to support automated extraction of syphilis data from EHRs as well as population-level querying using natural language.

WA Health Summary is a standards-based system that gives Washington State residents the ability to combine personal health data from healthcare organization and public health sources, to add their own information, and to electively share verified summaries. The system uses HL7 FHIR standards for the International Patient Summary and SMART Health Links, with links to EHRs, insurance, POLST and other systems, also benefiting public health. The precursor system is used by two million residents.
